Then I ask a simple question: What is your SHBG? Many patients have never heard of it because it was never checked.

Most testosterone does not travel freely through your bloodstream. Much of it travels attached to proteins.

One of the most important is sex hormone-binding globulin, or SHBG. Your liver produces SHBG, which binds strongly to testosterone. Albumin binds another portion more loosely.

Only a small percentage circulates as free testosterone.

This distinction matters because total testosterone tells us how much testosterone is circulating overall. Free testosterone describes the small portion that is not protein-bound.

I like to explain high SHBG and testosterone using what we call the Testosterone Availability Triangle™.

The three corners are simple:

  • Total testosterone: How much testosterone is circulating overall?
  • SHBG: How much testosterone is being tightly carried by this binding protein?
  • Free testosterone: How much testosterone is circulating unbound?

Think of total testosterone like money associated with a bank account. SHBG influences how much is tightly held. Free testosterone is the small portion that remains unbound.

The analogy is not perfect, but the lesson matters.

If we look at only one corner of the triangle, we may miss what the other two are telling us.

Major guidelines recognize this issue. The Endocrine Society recommends assessing free testosterone when total testosterone is borderline or a condition affecting SHBG exists.

The 2026 European Association of Urology guidance also recommends SHBG measurement and free testosterone calculation when SHBG-altering conditions are present.

Normal is a laboratory description. It is not the end of the clinical conversation.

Can High SHBG and Testosterone Therapy Explain Why You Still Feel Bad?

This is where the conversation becomes very practical.

I commonly see men with borderline testosterone and higher SHBG who report fatigue, poor motivation, low libido, erectile difficulties, belly weight gain, or slower recovery.

Some were told their testosterone was fine because their total testosterone fell inside the laboratory reference range.

Others are already receiving testosterone therapy. Their number has increased, but they still do not feel like themselves.

The problem is not that total testosterone is useless. It is an important measurement.

The problem is asking one number to tell us the entire story.

A Clinical Perspective: Persistent Symptoms Deserve a Deeper Evaluation

Imagine a patient who comes in exhausted. His libido has changed. Erections are less reliable. Motivation is down, and belly weight keeps increasing.

He tells me, “My doctor already checked my testosterone. It was normal.”

I hear this frequently.

When we look deeper, his total testosterone may be borderline while SHBG is elevated. Evaluating free testosterone can then provide information that total testosterone could not provide alone.

That does not prove testosterone explains every symptom. It tells us the original evaluation may have been incomplete.

This distinction is important because symptoms like fatigue, erectile dysfunction, poor motivation, and low libido are not unique to low testosterone.

Sleep apnea, thyroid disorders, medications, depression, metabolic dysfunction, inadequate nutrition, liver disease, and other conditions may produce similar symptoms.

The same thinking applies when testosterone therapy feels ineffective.

More testosterone is not always the answer to testosterone therapy that feels ineffective.

Before simply increasing a dose, I want to understand what we may be missing. That can include SHBG and free testosterone, but it should not stop there.

Sleep, nutrition, stress physiology, metabolism, recovery, medications, and overall health can influence how someone feels.

When patients discover that SHBG is elevated, they often ask me the same question.

“How do I lower it?”

I think there is a better question:

Why is it high?

SHBG is not the enemy. It is another piece of information.

Recognized factors associated with higher SHBG include aging, hyperthyroidism, certain liver diseases, some anticonvulsant medications, estrogen exposure, HIV, and genetic differences.

That is why blindly trying to lower SHBG can miss the point.

Better Hormone Decisions Begin With Understanding the Cause

Nutrition is another part of this conversation that I believe deserves more attention.

We cannot talk about hormones without considering how someone is eating, their body composition, energy availability, metabolic health, liver function, and thyroid physiology.

This does not mean there is an “SHBG diet.” There is also no single food or supplement that reliably fixes elevated SHBG.

Research shows why simple rules do not work.

A systematic review of seven randomized trials found calorie restriction affected testosterone differently depending on men’s body weight. In all four studies measuring SHBG, calorie restriction increased SHBG.

That does not mean calorie restriction is automatically harmful or that increasing SHBG is automatically bad.

It means context matters.

Consider two men with the same elevated SHBG.

One has adequate free testosterone, strong energy, good sexual function, and no concerning symptoms.

The other has borderline total testosterone, low free testosterone, fatigue, poor libido, and erectile difficulties.

The SHBG number may look similar. Its clinical meaning may be very different.

Higher SHBG is not automatically bad. Lower SHBG is not automatically better.

If testosterone therapy feels ineffective, the goal should not be chasing SHBG downward or testosterone upward. The goal is understanding the pattern.

Depending on the individual, that may include repeat morning testosterone testing, SHBG, albumin, and appropriately assessed free testosterone.

The Endocrine Society recommends two separate fasting morning total testosterone measurements when diagnosing hypogonadism. When indicated, free testosterone should be measured or calculated using reliable methods.

Patients can also ask better questions:

  • Was my SHBG ever measured?
  • Was my free testosterone evaluated appropriately?
  • Could something be raising my SHBG?
  • Have we considered my nutrition, thyroid, and liver health?
  • Could sleep or metabolic health be contributing to my symptoms?
  • Are we investigating why I feel this way, or only looking at my testosterone number?

Those questions can completely change the quality of the conversation.
